I think that its only the death eaters call him Lord Voldemort like to show respect, Harry calls him Voldemort to show hes not scared of him and but he may also call him Lord Voldemort for the same reason, but everyone else just calls him he-who-must-not-be-named. I think the Lord is in there to show his superiority.
